Precision Dents is required by law to maintain the privacy and security of your protected health information ("PHI"), notify you promptly if a breach occurs that may have compromised the privacy or security of your information, and follow the duties and privacy practices described in this notice.

  • Request access and copies of your dental records.
  • Request corrections.
  • Request confidential communications.
  • Request restrictions on how we use or share your information.
  • Receive an accounting of disclosures.
  • Receive a paper copy of this notice on request.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
